Fits five Alpha RD901-40 controls and brings the pins out to 15 holes on 2.5mm (.1 inch) centers. Can also accommodate toggle switches. The pre-drilled center holes make it work as a template if you are doing your own tooling. A companion 15-pin connector is available, or use Molex headers. Here is a layout drawing.
